Step 2: At the end of the application, there is no need to set up an appointment - Just come on over to complete your paperwork.

Please Bring the Following:

• Certified Birth Certificate

• Child’s Immunization Record

• 2023 Tax Return (1040 form) or 4 weeks of pay stubs

• Mortgage/Lease Document 

• Current Utility Bill

• Photo ID
