Ascension School-Based Health Center

CLPS, in partnership with Ascension Southeast Michigan Community Health, received a federal grant to create a school-based health center in Center Line High School. In this space, grant-funded staff from Ascension will provide mental health and medical services to all students living in the district, whether or not they attend our schools. All children who have paperwork on file will have access to the nurse practitioner and the behavioral health counselor, who will complete an intake assessment and then have the ability to call in any medical/psychological expert who may be needed to meet the needs of each child.  All children, regardless of whether or not the family has insurance, will be served. If a family has insurance, it will be used, but no co-pay will ever be collected.
Our goal at Ascension School Based Health is to improve the physical health and well-being of the students through our services at Center Line High School. 
 
Construction on the permanent physical location for the clinic is now completed, and the new health center is open for all children. We are open year round, Monday-Friday from 7:30-4pm. We are closed on holidays and weekends. Our Ascension healthcare professionals include Nurse Practitioner, Dr. Alyssa Clark DNP, FNP-BC; and Behavioral Health Counselor Mary McBratnie, MA, LPC, CAS.

To receive services, all students must have parental consent forms on file. Consent forms can be obtained from the Health Center. If you would like to schedule an appointment for your child or have any questions or concerns, you may call the Center Line Health Center at (586) 576-4038.
